What this list is, and what it is not
Read this before you rely on a revision date
- We do not host these forms. Every link goes to the state’s own file on its own website. Every one of them is free, and anyone charging you for the blank form is charging you for something the state gives away.
- 642 of 1123 rows carry a revision date. Where a state’s published list does not give one, we leave it blank rather than inventing it. A superseded revision is one of the commonest reasons a filing is sent back, so check the date on the form itself against the state’s site before you file.
- 69 rows have no form number because the state does not publish one for that document. They are listed by what they are. Another 38 are North Dakota packet steps — North Dakota numbers the forms within a packet rather than issuing form numbers, so “step 3” is a position, not a name a clerk would recognize.
